Forms & Resources

Forms to be completed before arriving in the US:

* Emergency release forms: to be completed by your parents and mailed back to Ronda Sheffield before your arrival. You can also give her the original documents upon your arrival.

* Immunization form :to  be completed by your parents and mailed back to Ronda Sheffield before your arrival. You can also give her the original documents upon your arrival.

* Flu shot shot authorization form be completed  and signed by your parents and mailed back to Ronda Sheffield before your arrival. You can also give her the original documents upon your arrival.

Important forms to read:

* St. Mary's Student Handbook to be read before your arrival.

Other forms:

* Overnight Permission Form to be completed and returned to Ms. Sheffield at least two business days before the overnight takes place.

* Day Outing Permission Form to be completed and returned to Ms. Sheffield at least two business days before the outing takes place.

 * Adopt an International Student Form to be completed by potential "adoptive parents" and returned to Ms. Sheffield.

* Adoption Program Handbook to be read, last page completed and returned to Ms. Sheffield.
